Types of Endometriosis Surgery: Knowing Your Options
Every patient is different and requires the different surgical approach. The right surgical approach for endometriosis treatment varies based on where the lesions are located, how far along things are, whether fertility is something the patient wants to hold onto, and their overall health picture.
Laparoscopic Surgery for Endometriosis
Laparoscopic surgery is widely regarded as the gold standard for treating endometriosis surgically. It’s minimally invasive, performed under general anaesthesia, and only involves a few small incisions, most often near the navel and lower abdomen. A small lighted camera called a laparoscope is passed through one incision, projecting a magnified view of the pelvic cavity onto a monitor. Instruments inserted through additional incisions are then used to remove or destroy any endometrial tissue found.
Within laparoscopy, there are two key techniques:
- Excision: The entire lesion, including its root, is cut out and removed. This is the more thorough approach and is associated with lower rates of symptom recurrence over time.
- Ablation (cauterization): The surface of lesions is burned or vaporized using heat or laser energy. It’s faster but may not address deeper tissue involvement.
Most gynecologic surgeons today favor excision for its completeness, though the right technique for you depends on the nature and distribution of your disease.
Hysterectomy
For women with severe disease who have completed their family planning, a hysterectomy, the surgical removal of the uterus, may be recommended. Sometimes the ovaries are removed as well, to stop the estrogen supply that keeps endometrial tissue growing. That said, it’s a permanent surgical call and not suitable for women who are still looking to have children.
