The Summer COVID Surge Is Back, and This Time It's Everywhere

If you've had a scratchy throat and a nagging cough over the past few weeks and told yourself it was probably just allergies or a summer cold, you're not alone, and you might also be wrong. COVID never really left, and this summer it's making itself known again, in a way that's a little different from the last couple of years. It's not confined to a handful of southern states anymore. As of mid August 2026, it's showing up basically everywhere.
What the numbers actually say
The CDC's own trend model, updated August 12, 2026, puts it bluntly: COVID infections are growing or likely growing in all 50 states. Not most states. Not the usual suspects. All fifty. That's a notable shift from where things stood back in July, when the CDC's forecasting models had infections rising in 27 states, mostly concentrated in the South and West.
Test positivity has been climbing too. Nationally it sat at 4.8% as of July 12, up 1.1 percentage points from the week before. Emergency department visits tied to COVID have been ticking upward across every age group, which matters because ER visits tend to show up in the data before hospitalizations do, usually by a week or two. The CDC's ensemble forecast for the week ending August 15 puts COVID-related ED visits at roughly 0.36% of all emergency visits, up from 0.25% just two weeks earlier. That might sound small, but the direction and the pace of the climb are what officials are watching.
Hospital admissions are following the same trajectory, just a little behind. About 1,700 new lab-confirmed COVID hospitalizations were reported for the week ending August 1. The CDC's model projects that number climbing to around 2,300 for the week ending August 15. To put that in perspective, back in mid-July the weekly admission count was closer to 1,200. So in the span of about a month, hospital admissions have nearly doubled, at least according to the projections.
None of this comes anywhere close to the numbers from 2020 or 2021, and it's worth saying that clearly so nobody panics unnecessarily. But the trend line matters, and right now it's pointing in one direction.
Meet Nimbus, the variant driving most of this
The strain behind the bulk of this summer's activity has a nickname that sounds more like a weather pattern than a virus: Nimbus, officially NB.1.8.1. It accounted for roughly 43% of sequenced COVID cases in the U.S. as of late June, and it's now the dominant strain nationwide.
Nimbus didn't start here. It first drove surges through parts of Asia earlier in 2026, hitting China, Singapore, and other countries in Southeast Asia before American airport screening picked it up in travelers arriving from places like China, France, Japan, the Netherlands, Spain, South Korea, Taiwan, and Thailand back in March. In Singapore, it reportedly spread at close to 60% weekly growth at its peak, which is fast even by COVID standards. Public health officials here used that Asian trajectory as an early warning sign of what might be coming for the U.S., and it looks like they were right to.
The World Health Organization classifies Nimbus as a "Variant Under Monitoring" rather than a full Variant of Concern, and both the WHO and the European CDC say the available evidence doesn't show it causing more severe illness than other recent Omicron descendants. The thing driving this surge isn't that Nimbus makes people sicker. It's that it spreads faster, and transmissibility, not severity, is usually what decides whether a summer wave turns into a real one.
There's also a second variant worth knowing about, one that's gotten less attention but has researchers paying closer notice: BA.3.2, nicknamed "Cicada." It's been picked up in wastewater samples across more than 30 states. What makes Cicada interesting, and a little concerning, is that it carries more than 70 mutations compared to earlier lineages, which raises real questions about immune evasion. As of earlier this year it hadn't caused a major surge anywhere in the world despite that mutation load, but researchers are watching to see whether that changes as it keeps circulating alongside Nimbus. And in parts of California, infectious disease specialists have also been seeing continued activity from XFG, nicknamed "Stratus," another Omicron-lineage variant that never fully went away.
Why summer, again
This isn't the first time COVID has picked the warmest months of the year to flare up, and the pattern has a fairly logical explanation behind it, even if it's counterintuitive at first glance. You'd think a respiratory virus would prefer winter, when everyone's already sick with something else and windows are shut. But COVID has behaved differently from influenza and RSV since it first showed up, and researchers increasingly think that's not really about temperature at all.
A few things line up every summer to create the right conditions. Immunity from whatever wave hit the previous winter starts to fade by June or July, so there are more people walking around with less recent protection. Summer travel picks up, putting more people from more places into closer contact. And, this is the part that trips people up, hot weather actually drives more indoor crowding, not less, because everyone retreats into air-conditioned spaces, malls, airports, offices, movie theaters, which creates the same kind of concentrated indoor exposure that normally happens in winter.
The CDC's own 2026 Summer Outlook flagged the South and West as the regions most likely to see early COVID activity this year, specifically because those areas had comparatively lower COVID circulation over the winter, leaving residents there with less recent immunity heading into warmer weather. That's exactly how things started. Wastewater data through the early summer showed high or very high viral activity concentrated in states like Alabama, Alaska, California, Delaware, Florida, Hawaii, Kentucky, Louisiana, and Texas. By mid-August, according to reporting that tracked the shift, the surge had stopped being a regional story. It's now rising in the Northeast and Midwest too, meaning there's no longer a "safe" part of the country where case activity is meaningfully lower.
Who's actually at risk
For most healthy, vaccinated adults, a Nimbus infection this summer is likely to look like a bad cold or a rough flu, a few days of misery and then recovery. That's consistent with what health officials have said about the variant's severity profile. But "most people will be fine" isn't the same as "nobody needs to worry," and there are groups where this still carries real risk.
People 65 and older remain the group facing the highest odds of serious illness. Data from COVID-NET, the CDC's hospitalization surveillance network, has consistently shown that the vast majority of adults who died after COVID hospitalization were 65 or older, and a striking number of those patients hadn't received an updated vaccine in a long time, in some tracked periods, most hospitalized adults hadn't gotten a COVID shot since 2023. People with underlying health conditions, the immunocompromised, and unvaccinated individuals of any age also face meaningfully higher risk, which is the same pattern that's held since the Omicron era began.
What's actually available to protect yourself
The vaccine situation is a little more complicated than it used to be, and it's worth understanding before you assume you can just walk into a pharmacy and get a shot. The current 2025 to 2026 updated vaccine targets the LP.8.1 variant, and it's available for adults 65 and older and for people ages 12 to 64 who have at least one underlying condition. Healthy adults under 65 without a qualifying condition generally can't get it through standard pharmacy channels right now, which is a shift from earlier in the pandemic when vaccination access was essentially universal.
Looking ahead, the FDA's vaccine advisory committee recommended in May 2026 that the 2026 to 2027 formula target the XFG lineage, the Stratus family of variants, for a vaccine expected to roll out starting in fall 2026. So there's a new shot coming, it's just not here yet, and it's being built around a different variant than the one causing most of this summer's trouble.
For treatment, Paxlovid remains the go-to antiviral for people at high risk, but timing matters enormously, it needs to be started within five days of symptoms showing up to actually help. There's also a newer option that's gotten attention recently: an oral drug called Xocova, which is being described as effective at preventing infection if taken within about 72 hours of exposure, aimed specifically at travelers and others in the 65-plus or high-risk categories who need a faster-acting option than waiting to see if symptoms develop.
The bigger picture
COVID is now more than six years past its original emergence, and this summer's wave is proof that it hasn't settled into the neat, predictable seasonal pattern that flu and RSV follow. Researchers studying this keep landing on the same conclusion, that SARS-CoV-2's timing depends less on the calendar and more on a mix of variant evolution, how much immunity the population has built up recently, and plain human behavior, travel, gatherings, indoor crowding. Until the virus settles into something more predictable, or until population immunity gets broad and current enough to blunt these waves consistently, summers like this one are probably going to keep happening.
None of this means canceling your plans or panicking over a sore throat. It does mean that if you're over 65, immunocompromised, or caring for someone who is, this is a good week to check your vaccination status and figure out whether you're covered, rather than finding out the hard way after you've already been exposed.
